These temporary membership cards are for you to give to new members when they sign up. There are ten perforated cards per sheet. Each has space for you to fill in the member's name and number.
The provider firm name and toll-free number will be automatically filled in on the front of all of the cards based on the state you select when ordering (unless you select the "Blank" option).
The back encourages your new member to download the mobile app.
